Care Partners seeks an MD or DO with board certification in palliative medicine, family practice, internal medicine, oncology or geriatrics to serve as our associate medical director 1-2 days/week. This position is responsible for supporting the interdisciplinary hospice and palliative care teams in providing high quality palliative end-of-life care in the home and community settings throughout the Portland, Oregon metropolitan area.   Responsibilities include providing outpatient and inpatient palliative care consultations, home visits for hospice recertification and symptom management, participation in our quality committee, and assisting with periodic physician call.

At Care Partners the associate medical director works as part of a team of prescribing clinicians including our Hospice Medical Director and nurse practitioner to care for a patient population that includes 60% with a primary diagnosis of advanced care. As a community-based, nonprofit reporting to a volunteer board of directors, Care Partners employees have the freedom to provide customized, innovative, quality care on a daily basis. Care Partners is a founding member of the Oregon Nonprofit Hospice Alliance (ONHA) which is assisting community-based, non-profits to advance their mission in the communities they serve.

As the Associate Medical Director you will help to shape the future of end of life care by participating in our efforts to expand home based palliative care, provide palliative care consults, facilitate goals of care conversations, mentor medical and pharmacy students, and develop ONHA.   You will work with a highly dedicated team of managers and staff to deliver innovative, high quality patient and family care and service that leads the market.  

 

  • Requires current medical license in Oregon
  • Requires good standing with Board of Medicine
  • Requires board-certification in palliative medicine, family practice, internal medicine or oncology
  • Requires ability to be credentialed for full scope of practice at partner hospitals
  • Eligibility and/or certification by the American Academy of Hospice and Palliative Medicine is desired
  • Requires proficient computer and electronic medical record skills
  • Requires practical experience and knowledge of medical ethics
  • Requires experience with quality improvement methodology
  • Requires good standing with Medicare such that non-administrative services provided can be billed.
  • Requires a valid driver’s license and access to a reliable automobile.
  • Requires ability to provide on-call services for face to face visits and telephone consultation
  • Requires successful completion of a professional background check and drug screen. 
  • Requires excellent communication, public speaking and interdisciplinary collaboration and leadership skills
  • Expert knowledge of hospice and palliative care strongly preferred
