VITAS Palliative Care Solutions, a division of VITAS Health Care Corporation, is seeking Highly Qualified palliative medicine professionals to oversee the daily operations and business development initiatives of a palliative medicine practice offering consultations to patients with serious, life-limiting illness.




Job Description
Serving Southern CA (San Gabriel Valley and San Fernando Valley areas)

As a Palliative Care Physician you will:

  • Assists with diagnosis, treatment and management of acute and chronic health conditions.
  • Attends and participates in internal VITAS Solutions palliative care team meetings.
  • Consults with the referring physician, the acute care team and other involved providers to contribute to the patient's plan of care.
  • Assists in development of clinical practice guidelines/ standards in support of quality palliative care as requested by supervisor.
  • Utilizes a customer service approach in all interactions with VITAS customers. Participates in on-call rotation as needed.

  • Provide consultation in palliative care, symptom and supportive care to meet the needs of the patient as requested by the referring physician.
  • Facilitate patient/family goals of care and the access to appropriate support models of care in hospice, rehabilitations, and on-going aggressive treatment.
  • Educate on all aspects of palliative care and the services provided.
  • Prescribe medications and treatments according to patients needs and in collaboration with the referring physician.
  • Order diagnostics/treatments as needed after collaboration with the referring physician and assists in development of clinical practice guidelines/standards.
  • Prepare and maintain accurate patient records to support the medical practice and reimbursement for services provided.
  • Adhere to the practice of confidentiality regarding patients, families, staff and the organization.
  • Supervise palliative care nurse practitioners and physician assistants.

Requirements
  • Hospice and Palliative Medicine board certified or board eligible preferred.
  • Must be a Doctor of Medicine or Osteopathy with two years experience, licensed to practice medicine in the state.
  • Advanced and ongoing training in palliative medicine, diagnostics skills, symptom management and communication skills.
  • Knowledge of the interdisciplinary team concept.
  • Reliable transportation, current state driver’s license and automobile insurance required.
