Job Description

General Purpose of Job:

 

Responsible for the administrative oversight and coordination of Hospice House and Palliative Care inpatient and outpatient programs. Plans and collaborates with referral sources and Care Providers for the provision and continuity of care for Hospice House and Palliative care. Serves as a liaison and educator for the clinical services directly related to the Hospice House and Palliative Care programs.

 

Essential Duties:

  • Assist in developing a vision and philosophy of care for the Hospice House and Palliative Care program which motivates the clinical staff to achieve operational goals and promotes referrals in the Mid-Michigan area.
  • Plays a leading role in facilitating referrals from other Mid-Michigan providers by developing and nurturing key relationships in the health care community that serves the terminally ill.
  • Provides leadership for the integration of the Hospice House and Palliative Care in the Hospice and Palliative Care Service line.
  • Serves as the key resource and manager for all clinical issues impacting Hospice House and Palliative Care.
  • Oversees the clinical staff practice of Hospice House and Palliative Care providers.
  • Screens, hires, orients and evaluates all Hospice House and Palliative Care Caregivers.
  •  Provides support to Hospice House clinical staff.  Supports effective administrative staff and policies that impact hospice clinical staff.
  • Attends interdisciplinary team meetings and develops staffing patterns for both the Hospice House and Palliative care programs.
  • Provides leadership and assists with the informed consent process.
  • Counsels patients, Caregivers and physicians through the transition to Hospice care.
  • Counsels team member and assist in determining appropriate placement or level of hospice care.
  • Monitors and is accountable for quality patient care as well as patient and family satisfaction.
  • Assists in establishing an annual operating and capital budget for Hospice House and Palliative Care.  Accountable to budgetary constraints.
  • Assist in development of policies and procedures that impact the clinical practices of Hospice House and Palliative Care caregivers.
  • Follows established HIPPA Privacy Procedures when using and/or disclosing protected health information.
  • Maintains and protects patients’ rights under the HIPPA Privacy standard.
  • Ensures that services of Hospice House and the Palliative care program meet regulatory and accrediting guidelines.
  • Serves as one of the principal spokespersons for Sparrow Hospice and Palliative Care, educating and promoting the continuum of care within the Health System.
  • Works collaboratively with the Sparrow Foundation to identify grant and assists in writing proposals for funding Palliative Care
  • In collaboration with the Hospice and Palliative Care team, analyzes data, identifies opportunities for improvement, coordinates action plan and implements changes
  • Participates and takes a leadership role in Hospice and Palliative Care fund raising events.

Job Requirements

General Requirements*Current Michigan license to practice as a Registered Nurse. *Hospice certification preferred *Nursing Home Administration preferred
Work Experience*Minimum 3 years Hospice experience. *Minimum of 3 years Palliative Care experience *Minimum 3 years management experience.
Education*BSN or MSW
Specialized Knowledge and Skills*Excellent documentation and communication skills. *Able to demonstrate the knowledge and skill necessary to provide care based on physical, psycho/social, educational, safety, and related criteria, appropriate to the Hospice and Palliative Care patient’s services, as related to the principle duties and responsibilities of the position. The skills and knowledge needed to provide such care may be gained through education, training or experience. *Interpersonal skills effectively and appropriately communicate with patients, families, physician, other Sparrow Health System Associates, and external agencies.
