Job Summary:

The Patient/Quality Care Coordinator Manager (PQCCM) must oversee and supervise the Patient Care Coordinators (PCC) who are responsible for the coordination of all aspects of patient care. The PCC has primary responsibility and authority for maintaining quality of patient care, in accordance with the organization's objectives and policies through planning, coordinating, implementing, and evaluating the services provided to patients and families.
