Palliative-Oncology Nurse Practitioner

Make a difference in the lives of patients in their families facing advanced cancer in beautiful Grand Junction, Colorado. HopeWest, an innovative leader in hospice and palliative care, is looking for an experienced Nurse Practitioner with exceptional communication skills to provide Palliative Medicine consultations for oncology patients in our care, serve as Attending for cancer patients receiving hospice care, help develop best practices around oncology and palliative care, and provide support to staff through education and provision of expertise.

Join HopeWest as a Nurse Practitioner and be a part of a team, working along side of

Dr. K. Ellissa Tiller MD FAAHPM CPE

Dr. Tiller joined HopeWest in December 2016 and serves as its Hospice Medical Director and Senior Vice President.  Prior to HopeWest, she established the Sentara Rockingham Memorial Hospital (RMH) Palliative Care Program in Harrisonburg, Virginia, 2010 and served as its Medical Director for over 6 years. Dr. Tiller was also the Medical Director for Sentara RMH Hospice. Dr. Tiller is Board Certified in Hospice and Palliative Medicine and was honored as a Fellow of the American Association of Hospice and Palliative Medicine in 2016.  She finished an Internal Medicine Residency program at the University of Virginia Roanoke-Salem program in 2000, and has extensive experience as a primary care physician and hospitalist. She completed medical school at the University of Queensland in Australia and worked as a general practitioner in rural Australia. Her awards include the Donnelly Award for best all-round Internist in 2000 and the RMH Golden Stethoscope Award for excellence in patient care, customer service, communication and teamwork in 2010, and the HopeWest Heart of Hope Award in 2018, for exemplifying the mission and values of HopeWest. She is recognized as a Certified Physician Executive (CPE).

Palliative-Oncology Nurse Practitioner

SUMMARY OF JOB RESPONSIBILITIES:

  • Works as a Nurse Practitioner, collaborating with Oncologists, Attending Physicians, HopeWest physicians/Nurse Practitioners and the Interdisciplinary Team.
  • Provides acute symptom management, facilitates goals of care discussions and discussions regarding advanced directives with patients and their families, and provides psycho-social support to patients and families facing serious illness from a cancer diagnosis in the following settings: private homes, Assisted Living Facilities, Group Homes, Nursing Facilities, Hospitals, office.
  • Assists in the development of best practices and protocols for hospice and palliative care patients with advanced cancer and acute symptom management.
  • Completes Medicare face-to-face visits for hospice re-certification.
  • Participates in on-call schedule with other HopeWest physicians and nurse practitioners.

QUALIFICATIONS

  • Board Certified Nurse Practitioner licensed in the state of Colorado.
  • Registered Nurse in the state of Colorado.
  • Unrestricted controlled substance license with current DEA number.
  • Must have and maintain hospice and palliative certification (CHPN or ACHPN) or obtain no later than the end of third year of employment. 
  • Oncology certification preferred.
  • Obtains and maintains CPR certification.
  • Minimum of two years of experience as a nurse practitioner, with oncology experience preferred.
  • Excellent interpersonal and communication skills.
  • Valid Colorado driver’s license and unlimited access to reliable transportation.
  • Adequate automobile insurance.
  • Eligible or currently possesses a Medicare/Medicaid UPIN number.
  • Eligible or currently credentialed at local hospitals.
  • Eligible to bill insurance for palliative care and symptom management visits.
