Transplant Clinical Coordinator Certified AdventHealth Orlando

Work Hours/Shift:
  • Full Time 8am-4:30pm
 

You Will Be Responsible For:   

  • Participates in 24hr on-call schedule, precepts/mentors new staff members, and actively participates in educational/promotional programs for transplantation.
 

The following responsibilities and core competencies may vary by organ group.


Pre-Transplant
:

  • Facilitates/coordinates individualized evaluations for potential organ recipients or living donors which includes: collection of patient medical data, patient/family education, identification of co-morbidities/contraindications to transplant (or living donation), collaboration with transplant team.
  • Communicates and collaborates with the appropriate multidisciplinary transplant team members and referring/treating physicians.
  • Maintains appropriate nursing documentation of evaluation process and progress.
  • Utilizes knowledge and is compliant with Organ Procurement Transplant Network and United Network for organ sharing (OPTN/UNOS) policies and requirements.
  • Coordinates care of candidate as needed while patient awaits transplant or living donation.
Peri-Operative:
  • Coordinates potential transplant surgery.
  • Communicates with patient, transplant physicians, surgical team, donor team, and hospital departments to facilitate patient admissions for transplant or living donations.
  • Maintains appropriate documentation of transplant surgery per OPTN/UNOS/AHTI policy.
  • Participates in interdisciplinary post-operative care planning and discharge plans.
  • Coordinates post-transplant patient/family education.
Post-Operative Outpatient:
  • Coordinates patient care in outpatient setting to achieve optimal healthcare recovery.
  • Reinforces patient education.
  • Monitors/assesses patient for potential post-transplant complications.
  • Assesses patient compliance with post-transplant plan of care and medication regimen.
 


Qualifications

What Will You Need:        

 

EDUCATION AND EXPERIENCE REQUIRED:

  • Critical care experience or palliative care of patients with life-limiting or life-threatening illnesses
  • Minimum of 12 months of experience in a solid organ transplant program
 

EDUCATION AND EXPERIENCE PREFERRED:

  • BSN or Bachelorâ™s degree in a health related field
  • Nursing leadership experience
  • Teaching/preceptor experience
  • Transplant care experience
  • Nursing case management experience
 

LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED:

  • Current valid State of Florida or multi state license as a Registered Nurse
  • Clinical Coordinator Transplant Certification (CCTC) from the American Board of Transplant Certification (ABTC)
  • BLS certification
 

LICENSURE, CERTIFICATION, OR REGISTRATION PREFERRED:

  • ACLS certification
 
Job Summary:

Transplant Clinical Coordinators are members of a multidisciplinary team that works together to provide individualized care to patients who are seeking and/or have received an organ transplant. Responsibilities include coordinating patient care through all phases of transplantation which include preoperative, peri-operative, and postoperative care, and follow up. Transplant Clinical Coordinators are also responsible for conducting professional and patient education programs related to transplantation. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.
