We're growing! Joliet Area Community Hospice, a non-profit organizations serving Will, Grundy, LaSalle, Kendall, Cook, DuPage, Kane, McHenry, Lake and a portion of Kankakee counties, seeks full time Field RN Case Managers.
We are currently seeking full time Field RN Case Managers in and around DuPage County to care for our patients. Our RN Case Managers have a caseload of patients that they visit according to the plan of care and are responsible for providing loving care for patients in their activities of daily living. These positions work Mon-Fri and must have availability to do approximately 15-20 hours of on-call per month.
Illinois Registered Nurse with a minimum of one year experience in either home health or nursing home/long term care. Previous hospice experience helpful
