NP/PA Palliative Care Full-Time Opening – Inpatient and Outpatient

Position Details:
Group is seeking an experienced NP/PA to join our new Palliative Care Division!
To be considered, candidate must have a minimum of three (3) years working as an NP or PA caring for a patient population with complex medical illnesses
Performs as an independent practitioner in collaboration with Palliative Care Physician
Inpatient and outpatient
5 days/week inpatient with no weekend call and 2-4 clinic sessions/week
Full-Time salaried position
Experience with advanced care planning strongly preferred
Duties of Position:
Provide coverage for other provider’s outpatient clinics
Perform initial and follow-up patient assessments and formulates treatment plans
Informs, explains and counsels patients and families
