$7,500 Sign-On Bonus - Registered Nurse (RN) Hospice Case Manager – The RN Hospice Case Manager works in collaboration with the Interdisciplinary Discipline Team (IDT) to foster a quality experience in accordance with the patient and family’s care plan and wishes. The RN Case Manager manages symptoms of the patient's disease process and re-evaluates effectiveness of interventions and adjusts the care plan as needed.

Shift: Monday - Friday, One Minor and Major Holiday with On Call as Needed
