The GEC Care Nurse Practitioner (NP) provides high-quality care to Veterans under the different clinics and programs (e.g. Home-Based Primary Care, Geri PACT clinic, Geri Assessment Clinic, Goals of Care Clinic and Outpatient and Inpatient Palliative services, etc.) as assigned. The NP in collaboration with the Medical Directors, and Program Managers of assigned services is responsible for the quality of care provided, productivity of the program, and for the development/implementation of programs and initiatives to meet the VA Salt Lake City Health Care System mission. The NP understands the Service priorities and objectives and facilitates accomplishment of the objectives in collaboration with the Clinic Manager, Program Managers/Directors, Medical Directors, and the Service Chief. In a
