Hudson Valley Hospice is a private, not-for-profit health-care agency serving the people of Dutchess and Ulster Counties by enhancing the quality of living for those at the end of life. When patients reach the end stages of an illness, Hudson Valley Hospice offers palliative care to help control pain and reduce anxiety. We improve quality of life and time spent with loved ones.

The Hudson Valley Hospice Weekend RN Liaison position is a hospital based role responsible responding to hospice referrals, collaborating with hospital staff to proactively identify patients who may benefit from hospice care, and working with patients and families to inform, educate and admit to hospice services.

As a Hospice Weekend RN Liaison, you will assess and evaluate patients for hospice eligibility. You will partner with physicians, case managers, and hospital staff to assist with symptom management and facilitate hospice admission and discharge planning. Since this Hospice Weekend RN Liaison role will work every weekend, you will be compensated at an enhanced pay rate up to $45/hour. In addition, as a part-time employee, you will be entitled to benefits, 144 hours of PTO (accrued bi-weekly), and holidays

ESSENTIAL FUNCTIONS

  • Review chart information and assess the patient’s clinical status
  • Use NHPCO guidelines to assist in the determination of eligibility for routine or inpatient hospice services
  • Provide hospice education and support for the patient, family, and members of the hospital team
  • Communicate and collaborate with all members of the interdisciplinary team on goals of care, patient status, discharge disposition etc.

PREFERRED EDUCATION & EXPERIENCE

  • Graduate of an accredited nursing school with a valid NYS RN license
  • Minimum 2 years clinical nursing experience
  • Previous hospice, home care and/or geriatric/chronic care experience preferred
  • Experience in discharge planning preferred
  • Current NYS Driver’s License required with acceptable driving record

WORK ENVIRONMENT/PHYSICAL DEMANDS
Employees are expected to successfully perform the essential functions of this job.

  • Working within assigned settings, including but not limited to Hudson Valley Hospice offices and hospitals,
  • Ability to carry up to twenty-five pounds of supplies and/or education materials and transfer/position an average adult for assessment and care.
  • Routine duties involve potential for direct exposure to blood or body fluids.
  • Ability to work flexible hours when needed

Hours: 30 hours per week (FRI/SAT/SUN OR SAT/SUN/MON)
Pay: Up to $45/hr based on RN experience
FLSA: Non-Exempt

  • On Call Evening Nurse - 40-50 hours per week (working between 4:30pm-8:30am; varying hours!)
  • RN Case Managers (Home Care) - Monday-Friday (8:30am-4:30pm)
  • RN Liaison (Hospital) - 4 10hr shifts - flexibility to work holidays and weekends
  • RN Liaison(SNF/Nursing Home) - Monday-Friday (9:00am-5:00pm)
  • Weekend Visit RNs - Saturday & Sunday (8:30am-6:30pm)
