Atlantic Health System is seeking a full time Nurse Practitioner to work in the Geriatric Program.

• NP will be responsible for post-acute care facilities in Morris county area
• Increase quality experience and patient experience
• The candidate will work closely with Geriatric physicians, nurse practitioners and the team to provide continuity of care and ensure transitions of care from acute care setting to long term or sub-acute setting.
• Function as an independent practitioner to communicate closely with nursing home staff and provide compassionate care to patients and families.
• Participate in after hours call schedule.

Candidates are preferred to have 2 yearsa experience working as an NP

Candidate will also have the ability to utilize the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served, through the model of Shared Governance, collaborate with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families, demonstrate and embrace the PRIDE cove values (Professionalism, Respect, Involvement, Dignity and Excellence) in every aspect of their job, and have excellent communication and computer skills.

Education:

• BSN required
• Masteras Degree required

Licenses and Certifications:

• Valid New Jersey Nursing License required
• BLS required
• ACLS required
• Geriatrics Certification preferred
