Position Summary:

The Hospice and Palliative Medicine Physician (HPMP) is assigned to the Hosparus Inpatient Care Center (HICC) or specific Hosparus Health community locations and is supervised by the Sr. Hospice and Palliative Medicine Physician and/or Medical Director.The HPMP provides direct patient care, including accepting patients as the hospice attending physician and providing consultations at the request of community physicians and community or Hosparus Health nurse practitioners, making medically-necessary visits (billable) and face-to-face recertification (FTF) visits (non-billable), when needed, and rotating to cover HICC night call, HICC weekend call and HICC weekend visits and/or general Hosparus-wide officer of the day, night call and weekend call. The HPMP supports and coordinates care with the Interdisciplinary Group (IDG) including participation in care planning meetings as the Medical Director’s designee and performs other necessary administrative functions, including certifications, recertifications, pre-authorizations and completion of medical forms and death certificates. The HPMP participates in education, outreach and leadership activities for the HICC or community locations when needed and may be assigned additional patient care or administrative duties at the discretion of the Sr. Hospice and Palliative Medicine Physician and/or Medical Director.

Position Duties & Responsibilities:

  • Provides education to Hosparus Health staff through informal consultations and educational offerings.
  • Provides outreach for Hosparus Health services to physicians and other healthcare providers.
  • Makes direct contact and performs service recovery with physicians who have complaints or misunderstandings regarding hospice care.
  • Performs peer review of attending physicians’ utilization of services.
  • Participates in development of clinical protocols and patient care guidelines.
  • Recruits and maintains community-specific Medical Advisory Committee membership if applicable.
  • Embodies the Hosparus Health values at all times.
  • Demonstrates clinical expertise in hospice and palliative medicine.
  • Maintains current state-of-the-art knowledge and skills in palliative medicine.
  • Works toward or maintains certification in Hospice and Palliative Medicine recognized by the American Board of Medical Specialties, or, if not board-eligible, maintains certification by a primary medical specialty board and works toward or maintains certification as a Hospice Medical Director.
  • Maintains necessary hospital staff privileges.
  • Maintains provider status with third party payors.
  • Provides medical consultation to hospice IDGs through attending regular care planning meetings.
  • Supports 24/7 medical coverage for hospice patients by participating in regular rotation in the HICC and/or general Hosparus Health on-call schedules.
  • Maintains accurate medical records and completes them in a timely manner.
  • Presents at and participates in Medical Staff meetings on a regular basis.
  • Provides consulting and attending patient care services, including medically-necessary visits with appropriate documentation, coding and billing capture, medical orders, prescriptions, family meetings, and coordination of care with hospice IDG staff and Hosparus Health or community physicians.
  • Participates in either HICC or Hosparus-wide general on-call rotations for urgent orders, prescriptions and telephone consultations.
  • Performs and documents certifications and recertifications in full compliance with Medicare regulations.
  • Completes required medical forms and death certificates in timely fashion.
  • Participates in admission, recertification, pre-authorization and utilization review decisions when needed.
  • Provides education to physicians, allied health care providers and general public in the community.
  • Participates in education, performance improvement, outreach and leadership activities, at the discretion of the Sr. Hospice and Palliative Medicine Physician and/or Medical Director.
  • Participates in building relationships with community physicians.
  • Participates in policy setting by attending quarterly meetings of the Medical Advisory Committee.
  • Participates in educating medical students, residents, and fellows.
  • Participates in Hosparus Health committees and work groups, at discretion of Sr. Hospice and Palliative Medicine Physician and/or Medical Director.
  • Provides education to community physicians, allied health care providers, and public in collaboration with Sr. Hospice and Palliative Medicine Physician and/or Medical Director.
  • Provides education to Hosparus Health staff through informal consultations and educational offerings.

  • Licensure (or eligibility for licensure) for medical practice in both KY and IN, including DEA and controlled substance permits, required.
  • Either fellowship in hospice and palliative medicine or a minimum of 2 years of post-residency medical experience required.
  • Hospice, palliative care, primary care, hospitalist, or geriatric experience preferred.
  • Board certification or Board-eligible with expectation to be certified within 5 years of employment required.
  • Certification in Hospice and Palliative Medicine preferred.
