VA Northern California Health Care System is looking for an enthusiastic candidate interested in providing effective patient care management in an interdisciplinary environment.

Duties/Responsibilities: The Geriatrics and Extended Care (GEC) Service is seeking a full-time Home-Based Primary Care (HBPC) physician in Sacramento, California. This physician will serve patients in the Sacramento Valley as part of an interdisciplinary specializing in care of frail, elderly Veterans, homebound Veterans, and Veterans with life limiting illnesses. The HBPC Program has been instrumental in providing outstanding services to Veterans with outcomes documenting significant reductions in readmissions, outstanding clinical performance measures, and leads the nation in Veteran satisfaction.

HBPC physicians are responsible for the medical care delivered by the HBPC team and assume primary medical responsibility for all patients in the HBPC program.  The incumbent is responsible for identification of the patient’s medical problems, defining the medical management of these problems, determining the need for consultation, and determining the need for admission to the hospital.  This individual attends and participates in weekly HBPC team meetings and is available to team members for collaboration when medical or other problems arise.  The HBPC Physician is responsible for patients assigned to his/her panel, and for cross-coverage during vacation, extended absences, etc of other HBPC Providers.  Upon admission, the physician evaluates the patient at home within 30 days, completing a full medical evaluation of the patient, attends all team meetings and participates in the formulation of the treatment plan. At the weekly team meetings, the physician participates in all 30 day, 90-day and annual reviews and signs the treatment plans used as Doctor’s Orders.  Incumbent will be available to staff during working hours via a cellular phone.  Incumbent works in the home care department, attends morning team huddles and informally meets with team members to review patient problems, sign orders, write new medical orders and review cases that the staff have seen during the day, etc.  The Physician is responsible for completion of performance metrics set by the VA-GEC-HBPC, participating in peer reviews and committee activities, and other projects. Academic appointments with UC Davis School of Medicine are encouraged.  We also have connection with Samuel Merritt University PA/NP students and other affiliations.

Candidate must be: 1) US Citizen or PRA, 2) Board Eligible/Certified in Internal or Family Medicine, 3) Have a full, unrestricted medical license in any State, and 4) confirm vaccination for seasonal influenza and Coronavirus Disease 2019 (COVID-19) which are both requirements for all Health Care Personnel (HCP) employed by Department of Veterans Affairs. Preferred experience: Highly qualified candidates will be Board Eligible/Certified in Palliative or Geriatric Medicine.
