The Home Based Program Coordinator (HBPC) is an extended care program at the New Mexico VA Health Care System in the Geriatrics Extended Care Line (GEC) that uses an interdisciplinary team, working with primary care providers to deliver healthcare services to patients in their homes, in concert with family and community support. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education and experience. The individual must meet at least one of the requirements below: (1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. (2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. (3) Doctorate degree in physical therapy. Licensure. Individuals hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Non-licensed PTs, who otherwise meet the basic requirements in this standard, may be given a temporary appointment as a graduate PT at the GS-11 grade level under the authority of 38 U.S.C. 7405 (a)(1)(D) for a period not-to-exceed two years from the date of employment on the condition that such PT provide care only under the supervision of a PT who is licensed. Failure to obtain licensure during that period is justification for termination of the temporary appointment. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-14 Physical Therapist (Supervisor, Service Care Line Manager or Service Chief): In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-13 grade level. In addition, the candidate must demonstrate all the following Knowledge, Skills and Abilities (KSAs): 1. Advanced knowledge of evidence-based practices and clinical practice guidelines in multiple professional areas. 2. Skill in managing interpersonal relationships and conflict resolution in dealing with a diverse range of employees, discipline lead(s), and administrators. 3. Ability to translate extensive rehabilitation knowledge into local policy development. 4. Ability to effectively supervise staff by providing clinical and administrative oversight. This includes ability to manage resources, evaluate employees and assign work. 5. Ability to collaborate with strategic planning committees at local, VISN or national levels for new ventures addressing patient care delivery systems, facilities management, system reorganizations, etc. References: VA Handbook 5005/99, Part II, Appendix G12 The full performance level of this vacancy is GS-14. Physical Requirements: The physical demands involve primarily walking, talking, driving, using the telephone etc. Strenuous physical activity is not required although the incumbent is expected to withstand pressure and stress as part of their day to day activities. This position may also required light lifting under 15 pounds, light carrying under 15 pounds, use of fingers, both hands required, walking up to 4 hours, standing up to 2 hours, climbing - legs only up to one hour, the ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16" to Jaeger 1 to 4, Far vision correctable in one eye to 20/20 and to 20/40 in the other , use of computer screens, and hearing aid is permitted. Census includes patients with multiple interacting chronic medical problems requiring prolonged intervention to maintain status and to retard decline, palliative care or terminally ill patients who are not ready for hospice but require home support, and short-term convalescent patients who need healthcare teaching and monitoring. The age range of patients may be from 17 to 100+ years but is predominately elderly. The incumbent provides the full range of supervisory duties which would include responsibility for assignment of work to be performed; performance evaluations; selection of staff, training of staff, and recommendation of awards, advancements, and when appropriate, disciplinary action. The incumbent coordinates development and implementation of the HPBC Program specific to HBPC staff assignment. The incumbent coordinates program implementation with multiple clinical services on campus and Santa Fe, Gallup, and Rio Rancho locations within the New Mexico VA Health Care System (NMVAHCS), including but not limited to nursing, physical therapy, occupational therapy, and social workers; and mental health and primary care providers. The incumbent may provide clinical interventions to the Veteran and caregiver populations served by the program. The HBPC Manager is also responsible for overall oversight of the HBPC Program and will manage and supervise all aspects of the program. This includes but is not limited to management and oversight of the Interdisciplinary Care Planning Process, Joint Commission standards, and ongoing assessment of home care services.
