Schedule: Full Time

Shift: Monday - Friday, 8:00am - 5:00pm

The community you’ll be caring for:

• Utilizes knowledge and skill in all LCSW/Oncology social work level appropriate psychosocial activities ad interventions.  Interventions include distress screenings, psychosocial assessments, illness-adjustment counseling, crisis intervention, individual and family counseling, concrete services/case management, and psycho-educational and therapeutic support groups.  Interventions include the development of treatment plans with patient and caregivers to enhance optimal psychosocial functioning during and after transplant treatment.
• Assesses patient’s and caregiver’s ability to understand diagnosis and treatments needs and works cooperatively with the treatment team in adherence to treatment recommendations.  Assesses patient’s functional, emotional and psychiatric status, provides clinical interventions and refers to mental health treatment when indicated.  Assesses patient’s and caregiver’s transportation needs, financial status, employment, disability and medication insurance with appropriate referral for medication and community assistance.
• Assesses patient and caregiver ability to comprehend and adhere to transplant care and participate in the development of treatment plans with patient and caregivers to enhance optimal psychosocial functioning throughout the diagnostic and transplant treatment continuum.  Assesses physical and emotional impact of transplant. Provides professional clinical competency and effective evidence-based best practice counseling in patient care specific to oncology, trauma, palliative care, and end of life, grief, and bereavement working within a multi-disciplinary collaborative team.  Conducts Depression Screenings and administers Suicide Assessments with a clear understanding of Florida Baker Act Law, to institute the Baker Act and/or work with appropriate physician to do the same, when necessary.
• Completes patient documentation and department statistics promptly.
• Works effectively under stress; managing crisis, conflict, and job challenges in a constructive manner and demonstrating self-care.

EDUCATION AND EXPERIENCE REQUIRED:

• Experience in oncology and hematology social work, or general medical social work and case management/discharge planning in acute healthcare setting
• Experience in clinical counseling, including palliative and end-of-life issues

LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED:

• Current active State of Florida license as a Licensed Clinical Social Worker (LCSW)
