The Coatesville VA Medical Center (CVAMC) is currently recruiting for a full-time Physician Assistant for the Community Living Center (CLC) located in the Geriatrics and Extended Care Service. BASIC REQUIREMENTS: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of a training program which is accredited by the Accreditation Review Commission on Education of PA (ARC-PA) or one of its predecessor organizations. Additionally, candidates must meet one of the following: Master's degree in any discipline; OR Bachelor's degree in any discipline with one year of experience as a PA. Certification: Must have successfully passed the PA National Certifying Exam (PANCE) Licensure: PAs must possess an active, current, full, and unrestricted license to practice in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. PAs must maintain current registration in the State of licensure if this is a requirement for continuing active and current licensure in that State. English Language Proficiency: PAs appointed to direct patient-care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d). EXPERIENCE : In addition to the basic requirements stated above, the following qualification criteria must be met: Physician Assistant-III (PA-III): Three years of successful practice as a PA; OR successful completion of a PA post graduate residency program sponsored by VHA Office of Academic Affiliations or other postgraduate residency programs accredited by VA, ARC-PA, US Armed Forces, or another accrediting agency AND one year of professional practice as a PA; OR a doctoral degree in a health-related field from an accredited college or university recognized by the U.S. Department of Education and one year of professional practice as a PA. PAs at this level should be able to demonstrate, develop, or acquire the following knowledge, skills, and abilities: Ability to participate clinically in the development of the PA patient care audit processes involving in-depth review and analysis of medical records. Ability to implement and initiate clinical changes to established treatment protocols based upon new medical literature research findings to enhance patient delivery care for improved patient outcomes. Ability to clinically facilitate the delivery of patient care in collaboration with health professionals, health managers, and other health care personnel. Ability to present clinical findings and recommendations to appropriate management officials. Skill to consistently demonstrate an elevated level of expertise and autonomy in diagnosing and treating seriously ill, multi-symptomatic patients through facility Focused Professional Practice Evaluation (FPPE) and Ongoing Professional Practice Evaluation (OPPE). Ability to clinically evaluate facility patient care delivery systems and validate or identify barriers to safe and effective patient care. PREFERRED EXPERIENCE: Specialized experience/training/certification in Geriatrics Medicine REFERENCES: VA Handbook 5005/134, Part II, Appendix G8, Physician Assistant Qualification Standard. This can be found in the local VA Human Resources Office PHSYICAL REQUIREMENTS: This work is mostly sedentary and requires some standing, bending and carrying light items. The incumbent must have the ability to stand and walk for prolonged periods of time to interact with the patients and other services. NOTE: Must have Basic Life Support (BLS) certification prior to assuming clinical duties. The BLS requirement is satisfied by successfully completing an American Heart Association (AHA) BLS Healthcare Provider Course or Military Training Network (MTN). BLS courses are normally offered within 1-5 days of entry on duty. No online certifications will be accepted. ["The Geriatric and Extended Care PA will provide high quality patient centered care to veterans under the different services such as Geriatric Medicine Outpatient Clinic, Community Living Centers, and Palliative/Hospice care service as assigned. The Physician Assistant, in collaboration with the Medical Directors and Program Managers of assigned services, is responsible for the quality of care provided by the program, for the productivity of the program and for the development and implementation of programs and initiatives to meet the MEDVAMC mission. Major duties include, but are not limited to: Provide patient care that is compassionate, appropriate, and effective for the promotion of health, prevention of illness, treatment of disease, and care at the end of life. Demonstrate the ability to independently manage a panel of patients with complex, chronic care issues using the best available evidence and national guidelines to achieve optimal population health and effective disease management. Demonstrate knowledge of established and evolving evidenced based biomedical, clinical and social sciences, and the application of their knowledge to patient care and the education of others. Appropriately orders diagnostic, therapeutic tests , pharmacologic and non - pharmacologic interventions relative to patient's age specific and health condition that support the ability to maintain or regain their health status. Ability to use scientific evidence and methods to investigate, evaluate, and improve patient care to ensure practice-based learning and improvement. Demonstrate interpersonal and communication skills that enable them to establish and maintain professional relationships with patients, families, and other members of the health care team. Demonstrates leadership and works collaboratively with assigned Primary Care Team and other interdisciplinary teams to identify, analyze and coordinate internal and external resources to resolve care issues (referrals/consults) and meet the medical and psychological needs or assigned panel of patients. Demonstrate behaviors that reflect a commitment to continuous professional development, ethical practice, and understanding and sensitivity to diversity, and a responsible attitude toward their patients, their profession, and additional duties as required by leadership. Provide education to patients and their families regarding the complex medical, social and behavioral conditions with a focus on self-management, prevention and wellness based on the patient's health goals and needs. Clinical Management including indirect patient care: open encounter & open consult management, CPRS documentation rules and requirements, adherence to clinic procedures for patient scheduling & access, ensuring staff productivity, oversight of relevant performance measures, and participation in the development of the new focus on the Cultural Transformation, Utilization, Quality, and Patient Safety oversight, process improvement (system redesign). Serves as key clinical primary point of accountability for patient involving Geriatric services (CLC and outpatient Geriatric Clinic) and collaborate with other providers and specialists in and outside of the VA system to manage patients' health requiring inpatient and/or outpatient care. Assessment, diagnosis, treatment and outcome measures of healthcare issues, histories, and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies for patient at home. Consults as applicable with other providers in primary and specialty care to assist in the care of geriatric patients as needed. Perform evidence-based practices to enhance symptom management of geriatric patients to include Palliative and end of life care when appropriate. Facilitate clarification and education to patient and family goals of care. Consults with the interdisciplinary care teams to establish a written plan of care and participating in interdisciplinary team meetings. Respond to critical lab/diagnostic test result notifications in a timely manner Participate in the FPPE/OPPE process. Adheres to employee health requirements.
