Landmark Health was created to transform how healthcare is delivered to the most medically vulnerable members in our community.  Our medical group provides home-based medical care to chronically ill patients, many of whom are ill-equipped to navigate our overwhelming healthcare system.  

Because many of our patients are frail and elderly, we deliver care primarily in the comfort of their home. Our Program is also offered to eligible patients at no incremental financial cost to them. We are not a fee-for-service practice; we benefit economically only if we deliver high-quality patient outcomes and satisfaction. As a result, our clinical teams can spend quality-time caring for a smaller number of patients, giving all patients the space, respect, compassion and care they deserve.

At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health, and other employed team members.

OBJECTIVE


Positions in this function consult with customers, members, patients, providers, internal departments and/or external third parties on any or all of the following:  Consult with and coordinate initial set up and on-going treatment plan for specialty drug patients, including DURs, co-pay assistance, ancillary products, scheduling Pharmacist consults and/or RN visits, and liasion with provider offices on labs, refill orders and prior authorizations. 

 

Primary Responsibilities:

  • Review and advise on medications and treatment plans (based on review of medical case notes, medication list and lab results) to improve outcomes and avoid potential drug interactions
  • Discuss with patients life issues affecting medication adherence and provide advice on improving drug regimen compliance
  • Advise patients on alternate Rx fill strategies of retail versus mail order and options for financial assistance
  • May outreach to providers on suspected medication abuse situations
  • Review medication regimen for disease state for LTC facilities/hospice care/RN case managers/Medical Directors/clients and provide summaries and guidance on future medication plans.  Advice may include alternate drug therapies, stopping a medication and/or lower cost alternative
  • Review medical case notes to determine on-going medication treatment plan and project future medication costs

Required Qualifications:

  • High School diploma or equivalent

 

Preferred Qualifications: 

  • Active/current National Pharmacy Technician Certification (PTCB, NCCT, or ExCPT) 
  • 3+ years of current/recent pharmacy or PBM experience 
  • Geriatric or Medicare Part D experience 
  • Ability to work in a fast-pace environment that is constantly changing 
  • Excellent telephonic customer service 
  • Basic knowledge of Microsoft Office Suite
  • Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Work Environment:
Below is a description of the work environment that the employee will encounter while performing the essential functions of the job.

  • Will travel as needed to office location in Saginaw
  • Noise level is usually moderate
  • Expected to work normal business hours between 8:30am-5pm Monday - Friday
