JOB DESCRIPTION

Primarily SNF work with some Hospice/Palliative care

• Lead a SNF team composed of a physician, NP, and Case Manager
• Average census 20-30 patients
• Round on skilled patients in the facility 3 to 5 times a week assisted by NP.
• Round on custodial patients for routine and acute visits assisted by NP
• Provide coverage for other physicians

Call: Weeknight 5-11pm twice per month - weekend call 8am-11pm once every month (cover both geriatric, and hospice/palliative care services)

Weekend Rounding: 1/2 day every other month- SNF admissions or urgent visits, hospital at home visits

No Memory Care or Geriatric Clinic in San Diego at this time

Hospital at home visits- upcoming opportunities to do in person and virtual visits
