Practice Description

Fairview Health Services is seeking a Nurse Practitioner at our M Health Fairview Clinic - Eagan location to be part of our Complex Care team.

Our team is focused on prevention of hospitalization and increased access to and integration with home care. Work alongside RNs, clinical pharmacists, and other team members dedicated to treatment of these complex patients.

Practice Details:

• Openings: 1.0 FTE (Full Time)
• 100% Outpatient
• 36 patient contact hours per week. Preferred schedule to be Monday through Friday.
• No call required.
• As a Pioneer Accountable Care Organization (ACO), our physicians use a multidisciplinary team approach in managing clinical care.
• Fully integrated Electronic Medical Record (EPIC).

Qualifications

• Graduate of a school of nursing and accredited Family or Adult Nurse Practitioner training program.
• BC/BE through the AANP or ANCC.
• Licensed or ability to obtain APRN license with the Minnesota Board of Nursing.
• Ability to obtain and /or maintain DEA certification for the State of Minnesota.
• Meet threshold criteria/qualifications for Credentialing and Privileges (if required).

Preferred Qualifications:

• Experience as Hospitalist or Primary Care provider
