Job Description

Advanced Practice Provider (NP/PA) Fellowship - Perioperative and Specialized Care

The MSK APP Fellowship Background

The Memorial Sloan Kettering Cancer Center's (MSK) Advanced Practice Provider (APP) Fellowship program provides Nurse Practitioners and Physician Assistants the opportunity to train at one of the top cancer centers in the United States. This Fellowship Program began over 40 years ago in Hospice and Palliative Care with one APP Fellow and now accepts over fifteen APP Fellows in six distinct disciplines. The Fellows are mentored by an interdisciplinary team of experienced oncology clinicians serving patients from around the world.

The MSK APP Fellowship Mission Statement

Our mission is to provide an oncology-intensive postgraduate academic program for the APP that promotes collaborative, high-quality, evidence-based, and compassionate patient-centered care to a diverse patient population in alignment with the institutional mission of MSK. The MSK APP Fellow will participate in clinical skills practicums, research, didactic learning, and clinical rotations on medical and surgical oncology and consultative services. Upon graduation from the Fellowship, the APP will be proficient in the delivery of exceptional comprehensive care of the oncology patient.

Advanced Practice Provider (NP/PA) Fellowship - Perioperative and Specialized Care

At Memorial Sloan Kettering, our Advanced Practice Provider-driven model in the

Anesthesia Pain Service (APS), Post-Anesthesia Care Unit (PACU), and Step Down Unit (SDU) allows APPs to autonomously manage patient care in collaboration with attending physicians for those services. The goal of the fellowship is to provide comprehensive training, which will be the key to forming well-prepared APPs in the specialty area. APPs will, in turn, be prepared to provide efficient, safe, and patient-centered postoperative and specialized care management to a diverse patient population.

The program consists of three, equal-length rotations with APS, PACU, and SDU. The fellows will be guided by experienced preceptors, managers, and the fellowship director.

Post-Anesthesia Care Unit (PACU)

The fellow will work collaboratively with Anesthesiologists, Certified Registered Nurse Anesthetists, Primary Surgical Teams, and Registered Nurses to efficiently recover patients who have undergone anesthesia. In this rotation, the fellow will focus on improving assessment skills to react to the patients quickly changing status. This clinical area treats side effects of nausea, increased pain, difficulty breathing, agitation, and those patients having trouble regaining consciousness so that the patient will have a stable transfer to the inpatient unit or discharge home.

Step Down Unit (SDU)

In this rotation, the fellow will obtain patient health histories, do comprehensive assessments, and give presentations during multidisciplinary rounds. There is an emphasis on formulating and implementing treatment plans for acute and chronic illness management in collaboration with Critical Care Medicine, Surgical and General Medicine Attending Physicians. The fellow provides direct clinical services and coordination of care for Critical Care Center patients

including mechanical ventilator management and interpretation of diagnostic/laboratory tests.

The Pain Service in the Department of Anesthesiology and Critical Care Medicine (APS)

The APS will offer APP fellows knowledge and experience in acute and chronic pain management. The fellows will acquire pain management understanding and clinical skills by rotating with the postoperative pain management APP group, inpatient chronic pain management group, and the outpatient chronic pain management clinic. This rotation focuses on interventional and medical pain management modalities, including patient-controlled analgesia, epidural and intrathecal catheter management, ketamine infusions, and spotlight evaluations for various peripheral nerve blocks.

Clinical Fellow responsibilities will include:

  • Obtaining patient health histories
  • Performing comprehensive physical assessments
  • Ordering and analyzing laboratory and diagnostic studies
  • Formulating and implementing treatment plans
  • Coordinating patient care
  • Evaluating the patient's response to treatment plans
