The LPN applies the principles of Patient Aligned Care Team (PACT), serves as the clinical associate of the PACT team let and works collaboratively with the expanded interdisciplinary team in the outpatient and clinic setting. The LPN observes, records and reports changes in the patient's condition and assists in the development and revision of patient entered plan of care. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
