Social Worker (MSW), Case Management Department
Overview Atlantic Health System is seeking a Part Time Social Worker ( MSW ) for their Case Management dept. Part Time, Day time shift with weekends and holidays on rotation located at Chilton Medical Center.Responsibilities Essential Functions:Provide social work coverage to facilitate discharges.Meet with patients and families to discuss and support the plan of care and engage them in discharge planning.Provide resources, referrals and support as needed on a case by case basis.Coordinator transitions of care to extended care facilities, inpatients behavioral health and chemical dependency facilities, hospice and palliative care programs, LTAC and Rehab.Attend daily huddles and interdisciplinary rounds and work closely
