About the organization

Santa Clara Valley Health and Hospital System (SCVHHS) is the second-largest County-owned health and hospital system in California and is committed to improving the health of the 1.8 million people of Santa Clara County.  As of March 2019, SCVHHS is comprised of three hospitals:  Santa Clara Valley Medical Center (SCVMC - a 731-bed central hospital), O’Connor Hospital (OCH - 358 licensed beds), and Saint Louise Regional Hospital (SLRH – 93 licensed beds); in addition SCVHHS includes  a large primary care network comprised of e eleven  health centers throughout the County, several urgent care clinics, a broad-range of specialty services in our Valley Specialty Center, comprehensive dental services, a large behavioral health department, public health, EMS, and Valley Health Plan.

Providers in our health system also have the opportunity to use our integrated electronic health record (Epic), which brings together system-wide patient information.   Recently, the Health Information Management Systems Society (HIMSS) recognized SCVMC for achieving its highest level of success (Stage 7), based on our continuous innovation and optimization of our inpatient and outpatient EHR.

About the Department

Our Post-Acute Care Transitions (PACT) Program is an award-winning initiative which focuses on patients who are medically stable and require nursing home level of care, but who do not yet have a safe discharge plan after hospitalization. Using a multidisciplinary approach, PACT coordinates a seamless transition for patients from the hospital to the nursing home where patients can access resources related to mental health, homeless medicine, substance use, wound care, housing programs, and palliative services. Once the patient is ready for discharge from the contracted nursing home, a team-based assessment informs a care coordination plan for patient integration back into the community.  We take care of our County’s high risk patient populations. We have the support of an excellent interdisciplinary team and committed physicians to ensure excellent career development.

About the Position

The Post-Acute Care Transitions Program is seeking a physician join our established team. You should have a current CA medical license, DEA and eligibility to participate in Medi-Cal/Medicare. The successful candidate will work closely with our robust interdisciplinary team to provide clinical care to our diverse, fragile patients, as well as coordinate transfers from the hospital, to the contracted SNF and facilitate safe discharges to the community. There is also opportunity for QI projects for the division as well as teaching Internal Medicine residents. This position also offers full time employment to work half time in Post-Acute Care Transitions and half time in Primary Care clinic.
