Provides assessment of psychosocial distress among outpatient oncology patients and facilitates counseling and referrals as indicated. Responsible for consultation and direction of social services, support groups, resource center, and oversight of ACS Programs and Services at the Benefis Sletten Cancer Institute. Identifies gaps in supportive care services for individual patients and their family and works within a multidisciplinary team to overcome barriers to patient care and facilitate access to resources for patients. Develops, organizes and implements programs, policies and procedures, conducts case work services and counseling and works with the SCI providers and administration to assure the needs of our patients are being met. Maintains performance improvement activities within the department and participates in CQI activities.

EXEMPT

Evaluates psychosocial distress for all newly diagnosed oncology patients and at appropriate intervals in care. Facilitates counseling or appropriate referral to supportive resources. Tracks and reports distress screening to SCI oncology committee.

Identifies and implements additional psychsocial measurement tools as needed for patient needs assessment.

Assures that social service/discharge planning activities are integrated and coordinated within the hospital system by effective communication at all levels.

Develops specific, measurable, realistic plans and objectives, which enhance social service delivery.

Ability to perform a psychosocial assessment and reassessment on all patients.

Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

Formulates a supportive care plan based upon identified needs and evaluates appropriate resources and/or referrals to satisfy those needs. Family is included as appropriate.

Oversees and manages the social resources needed by our patients, such as housing, transportation, financial assistance, and home care.

Demonstrates knowledge of clinical social work and discharge planning.

Treats patients and their families with respect and dignity. Identifies and addresses psychosocial needs of patients and family.

Oversees cancer support groups: assessing, measuring, improving, and identifying new opportunities.

Manages the resource center resources, working collaboratively with the physicians, nurses, and patient navigators.

Participates in public outreach, education, and Commission on Cancer activities as needed

Provides oversight for pastoral care services for outpatient oncology and assists with coordination of care between palliative care services and hospice when needed.

Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.

Communicates appropriately and clearly to physicians, staff, Nurse Executive and administrative team.

Coordinates and directs social services to ensure patients' needs are met

Provides appropriate resources and support to social services designee, both in the acute and skilled facilities.

Demonstrates an ability to be flexible, organized and function under stressful situations.

Maintains a good working relationship both within the department and with other departments.

Consults other departments as appropriate to collaborate in patient care and performance improvement activities.

Maintains performance improvement activities for department.
