ABOUT OUR HOSPICE TEAM | GOODWIN HOSPICE

Help us grow our team of seasoned hospice professionals! Goodwin Hospice, a service line of Goodwin Living, is a CHAP-accredited team that provides high professional standards in end-of-life care. We rank among the highest in Hospice Compare scores in Northern Virginia. The Goodwin Hospice team encourages continuing education, promotes self-care, supports team collaboration and respects all disciplines as we work to provide quality hospice care. 

POSITION OVERVIEW

The position is primarily responsible for the clinical management of a caseload of patients facing end of life and working with the interdisciplinary team to create a plan of care that meets the patient/family needs and goals.

JOB DUTIES

• Assumes primary case management responsibility for a patient/family caseload that includes the initial assessment, continued assessment, planning, implementing and evaluating all phases of nursing care.
• When needed, assesses patient needs and admission for service and follows protocol in admitting patients who meet eligibility criteria. 
• Re-evaluates the patient’s clinical needs at least every 14 days and is able to articulate continual eligibility, including documenting appropriate subjective and objective criteria.
• Initiates, implements, coordinates and documents the patient’s plan of care with the IDT in accordance with nursing needs specified in the approved plan of care.
• Delivers or supervises skilled nursing services using evidence-based knowledge regarding end-of-life care.
• Documents all clinical nursing visits, medication additions and changes, care plans, and additional documentation per protocol within 24 hours of provided service.

• Minimum of a bachelor’s degree.
• At least two years of administrative experience, preferably hospice or discharge planning.
• Is able to speak articulately about end-of-life issues and able to compassionately and calmly respond to clients who under duress.
• Proficient in Word, Excel and able to master documentation software competently enough to assist others in its use.

Inspired by this, Allen Adams (a layman serving on the home’s management committee) sought to create a similar, innovative community in Northern Virginia. With a generous donation of land in West Alexandria from Benjamin M. Smith in 1963, the plans to build the... first Goodwin Living campus were underway. When Goodwin House Alexandria opened in 1967, it was one of the first purpose-built continuing care communities in the country and the first in Northern Virginia. Twenty years later, following the success of Goodwin House Alexandria, a second community opened in Bailey’s Crossroads, a growing suburban neighborhood in Falls Church, Virginia. Goodwin Living Today Thanks to our incredibly history serving older adults in two Life Plan Communities, we have found ways to reach even more adults by take care and services beyond our campuses. Today, we offer a range of senior living and healthcare options, including our innovative Goodwin Living at Home program that empowers and enables older adults ages 55+ to remain in their homes for as long as they wish. We also offer Hospice, Home Health and Rehabilitation services to individuals throughout Northern Virginia, ensuring that anyone can thrive as they age in the places and ways they choose. In August 2022, we welcomed The View Alexandria by Goodwin Living (formerly Hermitage Northern Virginia) to our range of services. We’re proud of this legacy of leadership and innovation as we continue to reimagine and redefine aging, both within our communities and beyond their walls.
