St. Peter's Health Partners is seeking a full-time Nurse Practitioner or Physician Assistant to join Palliative Care Partners at Ellis Hospital located in Schenectady, New York.

Palliative Care Partners is a service line across St. Peter's Health Partners that supports patients and their families who are dealing with a serious illness.

The selected candidate will work closely with an interdisciplinary team including an MD, Social Worker, and Spiritual Care support to provide comprehensive care to patients with advancing illness primarily in the hospital setting. The candidate will work with consultant and practice staff to manage pain and symptoms, address goals of care and treatment plans and provide guidance and education to patients and families. We provide a supportive work environment and thorough orientation.

The schedule for this full-time opportunity is Monday - Friday, 40 hours per week.

We are seeking a candidate with a minimum of two years clinical experience, preferably in palliative care and home health, who has a passion for palliative care. Candidates must be Board Certified, hold a New York State licensure and DEA, and possess excellent clinical skills, communication skills, and a strong commitment to providing excellent care that is team oriented.
