Southwest Medical Home Health a home health division of OptumCare / United Health Group offers flexible shifts and the opportunity to make a difference in the care of patients in their home setting.

Primary Responsibilities:

• Provides direct care to patients under leadership of the RN
• Assures client driven plan of care meets positive outcomes through on going evaluation using evidences based practice
• Responsible for making certain the home care provided is compliant with State and Federal regulations (documentation and delivered) through participation in chart review and following established process
• Majority of job duties are carried out in patient's homes and driving throughout the community
• On call, weekends and rotating holidays as needed
• Shifts can include weekdays, weekends or combination of both

Required Qualifications:

• Current, unrestricted NV LPN license or temporary NV LPN license with the ability to obtain permanent NV LPN license upon expiration of temporary NV LPN license
• 1+ years of demonstrated competence in acute care, critical care, oncology, HIV / AIDS, advanced illness or medical / surgical patient care setting
• Willing to travel within Las Vegas providing direct patient care in the home setting
• Ability to lift up to 50 lbs and be able to perform these functions on a regular basis, such as sitting, standing, walking, bending, crouching, twisting and reaching
• Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
• Full COVID-19 vaccination is an essential job function of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements. Nevada Residents Only: The hourly range for Nevada residents is $20.00 to $35.53.
