The ACOS GEC provides administrative clinical and programmatic leadership for Geriatric and Extended Care (GEC) Line programs. Responsibilities include medical oversight of the Community Living Center (CLC), Home-Based Primary Care program (HBPC), the Advanced Illness/Palliative Care program, Geriatric clinics, and GEC Non-Institutional Care and Contract Programs. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Preferred Experience: 3-5 years of experience in Geriatrics Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: In Accordance With VA Directive and Handbook 5019 ["The functions and responsibilities for this position include but are not limited to the following: Provides direction and coordination for medical care in all GEC programs. Provides communication and education to residents/patients, family members, and significant others related to GEC care. Provides education to GEC staff and stakeholders related to geriatric programs and care. Promotes and supports a resident/patient-centered care model utilizing an interdisciplinary bio-psycho-social approach to medical care delivery. Provides consultation to the professional staff within all GEC programs and to community providers, concerning clinical assessment findings and appropriate interventions for resident/patients. Assures that primary medical care services are available to residents in the CLC. Assures that procedures are in place related to emergency treatment for CLC residents. Assures that procedures are in place to address resident transfers for higher level of care needs. Arranges and communicates to the appropriate staff Physicians, Nurse Practitioners (NP) and GEC administrative office availability and coverage. Coordinates the provision of clinical care to achieve optimal utilization of available resources. Ensures the availability and quality of GEC care provided in VA non-institutional and contract care programs. Assists in design, development, and implementation of clinical programs. Assists with the development of GEC components of the Medical Center Strategic Plan. Assists GEC leadership with program/policy development and changes to assure adequate, comprehensive service. Ensures the adequacy and appropriateness of the organization's scope of services, medical equipment, professional and support staff. Ensures annual C&P (Credentialing and Privileging) requirements are met by self and GEC licensed independent providers. This includes requirements for OPPE (Ongoing Professional Practice Evaluation) and FPPE (Focused Professional Practice Evaluation). Assists with the development of competencies for clinical staff, as well as assuring continuing education. Assists GEC leadership to assure that all programs meet standards issued by accrediting and regulatory agencies and the Veterans Affairs Central Office (VACO). Performs work related to the review, monitoring, analysis, planning and assessment of GEC programs. Assists and mentors GEC Clinical Program Coordinators in the execution of ongoing program evaluation and data-driven quality improvement activities to monitor effectiveness of clinical programs. Develops and monitors GEC standards of clinical practice/standards of care including development of care guidelines, manuals and guides to professional practice. Serves as member of ECMS, provides programmatic reports and updates to the VA FLHCS Executive Committee of the Medical Staff (ECMS) and ensures GEC program integration into strategic clinical initiatives Provides administrative and personnel management to GEC staff (including NP, Physicians and administrative staff); as such makes recommendations in matters such as promotions, selections, awards, EEO complaints, disciplinary actions, training, career development, and professional enrichment programs. Facilitates and liaises with other health care programs in order to effectively coordinate services. Functions as the FLHCS representative in the VISN REC ICC Oversees geriatric education experiences for FLHCS VA academic affiliation with GME programs Complies with national and local performance standards to meet facility performance measures. Provides medical care services for GEC clinics and programs as needed. Functions independently as a member of the GEC Medical Staff, with full clinical privileges. Assumes complete professional responsibility for clinical assessment findings, resident/patient-care decisions and documentation. Attends medical staff meetings, assures adherence to its by-laws, and participates in the Peer Review process. Adheres to policies and procedures established by VACO, VISN2, and the local Medical Center. Provides after hours, weekend and holiday calls with other physicians for CN CLC Provides after hours, weekend and holiday calls for urgent administrative issues related to FLHCS GEC programs Work Schedule: Monday - Friday 8-430pm"]
