Alameda Health Medical Group (AHMG) is recruiting for an experienced Board-Certified Palliative Care Physician to expand community-based services, including adding services in post-acute (skilled nursing) facilities and patients’ homes, and expanding outpatient clinics. This physician leader will join Alameda Health System Palliative Care Services, a well-established interprofessional team that provides high quality inpatient and outpatient support for patients who are experiencing serious illness in the Alameda County safety-net.

Location: Oakland, California

Schedule: 8-hour shifts, Monday-Friday

Reports to: Chief, Division of Palliative Care

Core Job Responsibilities

  • Lead development of palliative care in Alameda Health System post-acute facilities, including collaborating with clinicians working at each site to identify needs and plan services, establishing interprofessional teams and procedures to perform consults and provide support at post-acute facilities, establishing quality metrics, and developing procedures for documentation and billing.
  • In conjunction with the current palliative care community-based team and clinic medical director, assess needs for and plan additional expansion of community-based palliative care services, including services offered in AHS wellness centers, virtually, at non-Alameda Health System post-acute facilities, and in patient’s homes.
  • Serve as Associate Chief for the Division of Palliative Care, including supporting service leadership and covering for Chief of Palliative Care when Chief is not available.
  • Provide Inpatient Palliative Care Consultations at Highland Hospital, in collaboration with an experienced and well-established interprofessional team (Chaplain, Physician assistant, Social Worker)
  • Provide Community-Based Palliative Care in person, via telehealth, and in skilled nursing facilities in collaboration with an experienced Palliative Care Social Worker, Chaplain, and Outreach Worker
  • Provide Inpatient Palliative Care Consultations at the two smaller hospitals within Alameda Health System: Alameda Hospital and San Leandro Hospital, in collaboration with interprofessional teams (Chaplain, Nurse, Nurse Practitioner)
  • Symptom management, serious illness communication, psychosocial and spiritual care support, linkage to community resources, tailored to the needs of the diverse and vulnerable population we serve
  • Collaborate with providers from a range of medical and surgical specialties and subspecialties who share a commitment to serving vulnerable populations
  • Collaborate with other palliative care program team members to lead educational sessions, including communication skills training and symptom management workshops
  • Provide clinical teaching to residents in Internal Medicine, Emergency Medicine, and General Surgery, equipping them with an approach to patient and family care that highlights cultural humility, psychosocial and spiritual care, and self-reflection to promote connection and resilience
  • Partner with other specialties and subspecialties to provide education to their residents and staff and implement quality improvement projects
  • Other responsibilities as designated by Chief of the Division

Required Qualifications 

  • Doctor of Medicine or Osteopathy
  • Subspecialty Board certified in Hospice and Palliative Medicine
  • California state medical license and DEA license
  • Experience in palliative care program development and service expansion
  • Experience in community setting, including skilled nursing facilities and/or home-based care
  • At least 5 years working as a palliative care physician
  • Commitment to practice in an interprofessional team in the safety-net setting, serving a linguistically, ethnically, and socioeconomically diverse population

Preferred Qualifications 

  • Fluency in language(s) frequently spoken in the Alameda Health System patient population: especially Spanish, Cantonese, Mandarin, Vietnamese, Tagalog
  • Experience managing interdisciplinary direct report personnel
