This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Physical Requirements: In Accordance With VA Directive and Handbook 5019 ["This position is for a Geriatrician (CLC Medical Director) with responsibilities for HBPC and GEM Clinic The Finger Lakes Healthcare System at Bath, NY is recruiting for an experienced and highly qualified Board Certified/Board Eligible physician for the Geriatrics Extended Care and Rehabilitation (GEC&R) care line to provide oversight of our CLC, Palliative/Hospice Care, Home Based Primary Care (HBPC) and Geriatric Evaluation and Management (GEM) program(s). The incumbent will provide direct patient care, offer support to the provider staff and work with additional interdisciplinary staff. He/she will work closely with the Associate Chief of Staff (ACOS) of the GEC&R Care line, Associate Chief Nurse for GEC&R at the site and collaborate with the GEC&R Care Line Manager. The successful candidate should have advanced clinical experience as well as demonstrated administrative skills and a track record of dynamic, innovative leadership that emphasizes a collegial approach. This physician functions as a subject matter expert for educating Veterans, staff, and family about geriatric syndromes, functional assessment, and end-of-life issues. This will include, but is not limited to, documentation, process workflow and other functionality regarding necessary clinical systems. This individual will work with staff and physicians to optimize the usage of clinical systems to promote quality outcomes and provide support in accordance with organizational goals. He/she will work closely with hospital staff, department directors, and physicians to ensure that the highest standards of quality and services are maintained. He/she also serves as a liaison between customers and other departments and services and will work directly with all levels of the organization on projects, committees and teams to meet the overall needs of GEC&R and the facility. The individual should possess clinical experience, be familiar with a variety of the field's concepts, practices, and procedures and possess experience and judgment to plan and accomplish goals. The individual will also perform as a back-up provider for the Hospitalist/Medical Officer of the Day (MOD) and placed in a standard rotation for coverage when needed. This job requires knowledge of administrative procedures, quality measurement, regulations related to the practice of medicine, Long-term Care Standards, Joint Commission, and other regulatory standards pertinent to GEC&R. Additional duties: Admit, follow and discharge Community Living Center (CLC) patients as needed to maintain good patient flow- Provide geriatric and palliative care consultation for the Veteran population. Provide clear, complete, direct and electronic communication with staff in all areas of the Finger Lakes Healthcare System and with community contacts to facilitate smooth patient transitions, quality patient care and high customer satisfaction. Participate in interdisciplinary team meeting (IDT) weekly and care plan/family meetings in conjunction with clients' care needs Meet ethical and professional standards as written in the Medical Staff Bylaws and ensure that work products are in compliance with written and communicated facility expectations, standards, policies and procedures Provide informal clinical consultation for the GEC&R staff- Participate in selected VA committees as needed- Maintain current requirements for credentialing and privileging including necessary license and certifications Pursue high quality continuing education and professional development opportunities Participate in appropriate regional and national professional organizations Duties require numerous daily contacts with patients, employees and visitors, in addition to VA headquarters representatives, VISN representatives, medical care representatives, officials of other government agencies, representatives of our educational affiliates, community and state health care providers, etc. Develops and monitors GEC&R standards of clinical practice/standards of care including development of care guidelines, manuals and guides to professional practice. Develops and monitors GEC&R staff training to ensure that required training is accomplished for purposes of maintaining ongoing skills, keeping up with new developments in the field, orientation of staff to the VA, The Joint Commission and Long Term Care Institute requirements, licensing requirements, and other VA required training. Reviews the utilization of resources by performing such activities as internal reviews, data collection, reviewing and analyzing reports, reviewing contracts and reviewing performance in order to ensure that resources are being used efficiently and effectively, and reports results to management as required, both orally and in writing.
