Nemours is seeking a Palliative Care and Bereavement Counselor for Wilmington, DE location.

Primary Functions:

Purpose of the Position:

1. Help coordinate hospital bereavement services and serve as a resource for staff and bereaved families.
2. Develop new and augment existing bereaved family programming in conjunction with Palliative Care Child Life Specialist and Palliative Care Chaplain
3. Provide skilled assessment and intervention for children with chronic, complex and/or life-threatening conditions and their families who have palliative care and/or bereavement needs when requested.
4. Provide clinical consultation as an active member of interdisciplinary care team.
5. Attend to system dynamics of the health care delivery network to foster best use of resources, effective service delivery, and patient and family satisfaction.
6. Ensure compliance with practice standards and codes.
7. Advocate for interventions that alleviate burden and facilitate coping for PPC and bereaved families.

Essential Functions:

BEREAVEMENT (85%):

• Develop, augment and coordinate all bereavement programming for Nemours families in the Delaware Valley.
• Offer family counseling, utilizing cognitive behavioral therapy and other appropriate modalities to facilitate effective coping with anticipatory grief or bereavement.
• Arrange referrals to appropriate community services and resources in collaboration with the interdisciplinary team.

PALLIATIVE CARE (15%):

• Perform and document thorough palliative care psychosocial assessment from a family-centered care perspective.
• Facilitate communication among family and team members.
• Negotiate areas of service overlap and gaps with other mental health professionals who are active in the care of palliative care patients.

OTHER

• Participate in conflict management, bereavement care planning and support, and training and education of hospital staff.

• Participate in continuous improvement and quality improvement activities as assigned or indicated.

• Other duties as assigned - Additional miscellaneous duties and responsibilities, as may be assigned from time to time by employee's supervisor.

Qualifications:

1. Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC) or similar
   • Must remain actively engaged with clinical supervision and Preferred: Maintain a personal counseling/therapy relationship.
2. Demonstrated experience working with bereaved parents/families in a clinical capacity; background in bereavement following traumatic death strongly preferred
3. Demonstrated experience working with life-threatening conditions, chronic disease, hospice and / or end-of-life care, and related situations
4. Demonstrated experience working with family systems
5. Licensure appropriate to the State and organizational guidelines for location of practice

Job Requirements:

Master's Degree required.

Licensed Clinical Social Worker (LCSW) or Licensed Professional Counselor (LPC), CT (Certified in Thanantology) or similar.

1-3 years of job-related experience

Must remain actively engaged with clinical supervision and Preferred: Maintain a personal counseling/therapy relationship.
