The RN demonstrates leadership in delivering holistic care through collaborative strategies with others (VA Handbook 5005, Part II, Appendix G6), proficiency in decision-making and using the nursing process for complex situations, as well as collaboration by using the group process to solve problems. The focus is on leadership in strategically planning long-term goals for the microsystem (unit, team, or workgroup) that impacts operations or resources. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles. ["VA offers a comprehensive total rewards package: VA Nurse Total Rewards The Outpatient Spinal Cord Injury and Disorders (SCI/D) Nurse practices as a Registered Nurse (RN) in the SCI/D center that specializes in: 1) efficient delivery of rehabilitation; 2) sustaining medical and surgical care; 3) patient and family education; 4) psychological, social, and vocational care; 5) research; 6) education; and 7) professional training in the continuum of care for persons with SCI/D. Patient populations served include a full range of care for all enrolled Veterans who have sustained an SCI or have a stable neurologic impairment of the spinal cord. Typical patient populations served include SCI, Amyotrophic Lateral Sclerosis (ALS), Multiple Sclerosis (MS) as outlined by SCI/D VHA Handbooks and Directives. The incumbent will triage the unscheduled needs of this population. Before an appointment with the provider, the nurse will review with the Veteran their existing problem list and any new or reoccurring issues. The RN obtains the Veterans weight and vital signs, assesses skin, and assesses bowel and bladder issues. The RN assists the Veteran and VA staff regardless of location (e.g. HBPC, inpatient stay, nursing home, home, etc.) in troubleshooting medication and supply issues, skin care assessment, review of SCI practices and protocols, referrals to the SCI/D Center, SCI/D telehealth visits, care planning, telephone triage, autonomic dysreflexia risks, fee basis, bowel and bladder, etc. This patient population is likely to present with complex conditions that may require acute stabilization; acute and sub-acute rehabilitation; acute and sub-acute medical and surgical care; preventative care; respite care; hospice care etc. The RN is accountable when assigned as a primary nurse for a defined group of patients for the entire episode of care, shift or clinic appointment. Based on education and experience, may be assigned responsibilities as preceptor, charge nurse, or unit shift leader. The RN provides age-specific care for adult (18-64), older adult (65-74) and geriatric (over 75) patients who have sustained damage to the spinal cord throughout the continuum of their lifetime, which incorporates stabilization after injury, rehabilitation, unrelated medical Vs surgical conditions, health maintenance and long-term follow-up of the injured individual, as well as psychosocial support including the family of significant others. The RN promotes achievable independence and adjustments conducive to health, assisting the patients and their families to look beyond the disability. Their supportive interventions reduce the stigmas of disability and help individuals reestablish and maintain control over all aspects of their lives. The RN collaborates with interdisciplinary team to promote efficient, effective, individualized care to assist the patient in achieving optimal function and health. Members of the interdisciplinary team vary depending on the needs of the patients. The RN promotes the VA Mission, Core Values, Strategic Goals and Objectives. Additionally, the RN upholds the Mission of the Spinal Cord Injuries and Disorders program in VA to support and maintain the health, independence, quality of life, and productivity of Veterans with spinal cord injuries and disorders throughout their lives. The RN will: apply the nursing process to systems to improve care; demonstrate leadership by involving others in improving care; support client self-determination; serve as a resource for clients and staff in resolving ethical issues; identify resource utilization and safety issues taking appropriate action; acquire knowledge to maintain expertise in areas of practice; participate in educational activities to improve clinical knowledge and enhance role performance; evaluate practice of self and others using professional standards, relevant statutes, and regulations, and take action to improve performance; use the group process to identify, analyze, and resolve care problems; educate colleagues and/or students and serve as a preceptor and/or mentor; initiate/participate in quality improvement activities; use a body of research to validate and/or change work group practice.
