Physician - Home Based Primary Care
Location: Merrifield, VA - HeadquartersSchedule: Monday - Friday - 8:30am to 5:00pm GENERAL DESCRIPTION SUMMARY The Capital Caring Health Home Based Primary Care Physician will join an established house calls practice providing home based primary care to patients who are home limited. Care will be provided in independent homes and senior communities (including both assisted living and independent living). The applicant will join an established team consisting currently of 2 physicians, 2 nurse practitioners, social worker, triage RN, and 2 office managers.
