Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Additional Requirement: Must be certified by the American Board of Internal Medicine (ABIM) or American Board of Family Practice (ABFM) and hold an unrestricted license to practice in at least one state. Preferred Experience: Palliative Care Board certification preferred Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: :Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting. ["The primary role is to the Palliative Care Program of the Geriatrics and Extended Care program. The physician practices in a complex patient care setting in collaboration with interdisciplinary colleagues. The role includes Palliative Care consultation, both inpatient and outpatient, for veterans with serious illness. The incumbent may cover the Outpatient Geriatric Clinic with a multidisciplinary team with diagnose and treat diseases affecting the elderly, and also frequently work to improve the standard of living during the natural progression of aging. Incumbent must have in depth knowledge in both serious illnesses and long-term illnesses affecting veterans, including the elderly, such as dementia, COPD, cancer, and heart disease. Incumbent must be highly knowledgeable in the most common complaints/results of the aging process, including incontinence, reduced memory, physical weakness, infection, and immobility. Additionally, they must be educated in, and consider the psychological effects of aging as well as symptom management, which includes pain management. Incumbent should also know about goals of care discussion, end-of-life legal issues, such as wills, power of attorney, etc., and they must carefully work with other physicians as much as possible so medications are regulated, medications are not over-prescribed, and combinations of many medications do not cause harmful reactions in patients. Experience in teaching and/or desire to help develop and grown Tomah's Palliative Care Program . FUNCTIONS Oversight of daily operations, and provide clinical, programmatic and administrative leadership to the Hospice and Palliative Care service. Collaboration with health care partners to measure, track, and respond to quality metrics. Provide physician services, clinical input and oversight for all patient treatment plans consistent with hospice and palliative care philosophy. Provide inpatient and outpatient consultation services in hospice and palliative care. Be an active participant and facilitate clarification and the development of patient and family goals of care. Improve access to Palliative care and Hospice as experienced by patients, their families and providers; including community hospice partners. Amplify the awareness of the concepts, role and availability of Palliative Care and Hospice services among ambulatory, home-based, specialty and acute care clinicians. Facilitate access to appropriate supportive care services. Consult with attending staff as requested so that symptom management for patients is effectively and expeditiously accomplished. Apprise the Palliative Care and Hospice team of medical care and practice standards Plan and direct educational experience for medical students, residents, and Nurse Practitioners assigned to Palliative Care and Hospice if available. Provide counsel to the Community Living Center (CLC) leadership team regarding process improvement, program development, and policy decisions related to Palliative Care and Hospice services. Train ambulatory, specialty and acute care clinicians to provide basic palliative care through improvements in service delivery policies, procedures and interfaces. Develop and provide education to medical center staff and coworkers. Full and appropriate documentation of patient care services. Pursue opportunities for partnering with other specialties, including Primary Care, Mental Health, Home Based Primary Care, and Neurology within the facility; and Madison specialties as indicated. Flexibility and a compassionate bedside manner are essential Skills in team building are essential Board Certification in Hospice and Palliative Care is preferred.
