OPPORTUNITY HIGHLIGHTS

LIFE St. Joseph of the Pines is seeking a part-time, experienced Nurse Practitioner to join our Trinity Health PACE program located in Fayetteville, North Carolina.

The primary role of the NP is to provide primary medical management in collaboration with the PACE Organization (PO) physicians.

Responsibilities include:

  • Performs assessments in collaboration with the physician and as needed. Develops and implements appropriate plans of care to PO participants.
  • Evaluates participant physical complaints and provides appropriate treatment.
  • Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT).
  • Provides daily oversight of PACE Organizations Clinic.
  • Perform rounds in the POs contracted nursing homes.
  • Regularly informs the IDT of the medical condition of each participant, remaining alert to pertinent input from other team members, participant's caregivers, as well as documenting changes in a participant's medical record consistent with documentation policies.
  • Participate in provider on-call rotation as required.

Requirements:

  • Master's degree in Nursing with five (5) to seven (7) years of experience required, preferably in long-term care.
  • Must possess a current, unencumbered, active license to practice in the state of North Carolina.
  • CDS Certification and DEA Registration required.
  • Board Certification by ANCC or AANP.
  • Certification as a Nurse Practitioner with prescriptive authority required.
  • Current BLS required.
  • A minimum of one (1) year working with the frail or elderly population strongly required.

St. Joseph of the Pines (SJP) is a nationally accredited aging services network offering a full continuum of retirement housing, health care and community-based services for older adults as well as community outreach to those in need.

St. Joseph of the Pines (SJP) is committed to promoting emotional, physical, and spiritual well-being to those in the organization's care. Programs and services include continuing care retirement, assisted living services, skilled nursing, rehabilitation, and memory support. In addition, SJP's extensive network of care offers Home Care, affordable housing, a Program of All-Inclusive Care for the Elderly (PACE), and a rural mobile health clinic.

Living Independently for Elders (LIFE) is a Program of All-inclusive Care for the Elderly (PACE) at SJP, which assists older adults in living safely at home. Eligible participants are 55-years old or older, live in the designated service area, meet the state requirements for nursing home care and are able to live safely in the community.

LIFE is designed around the needs of the individual and provides comprehensive medical, health and social services (including adult day health programs, rehabilitation therapy, nutritious meals and social engagement) to participants at the center as well as in their homes. The growing program currently supports 220+ participants and is expecting expansion efforts in the near future.
