This position is responsible for Neurology patient care coordination for multidisciplinary clinics and research. The position is integral in navigating the requirements of an ALS Coordinator. Position will report to the Neurology Service Chief. Position is responsible for planning and carrying out the assignments independently, resolving most of the conflicts that arise, coordinating the work with others as necessary, and interpreting policy on own initiative in terms of established objectives. To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 09/14/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-9 position you must have served 52 weeks at the GS-7. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. BASIC REQUIREMENTS: Must meet one of the following: - Have a degree in behavioral or social science; or related disciplines appropriate to the position. - Have a combination of education and experience that provided me with the knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. - Have four years of appropriate experience that demonstrates that I have acquired knowledge of one or more of the behavioral or social sciences equivalent to a major in the field. GS-9 GRADE REQUIREMENTS: Must meet one of the following: Specialized Experience: Must have 1 year of specialized experience equivalent to the GS-7 grade level in federal service that equipped you with the knowledge, skills and abilities to perform successfully the duties of the position, and that is typically in or related to the position to be filled. Examples of specialized experience would typically include, but are not limited to: Communication and interpersonal relations, including the ability to appropriately and courteously relate to internal and external customers. Maintenance of confidentiality of patient/incumbent information, electronic and print. The Hospital performance improvement program. How to work and function in the Hospital in a safe manner, including infection control and the correct operation of equipment. OR, Education: Must have 2 years of progressively higher level graduate education leading to a master's degree, or master's or equivalent graduate degree. Combination: Must have a combination of higher level graduate education and specialized experience as described above. You will be rated on the following Competencies for this position: Administration and Management Customer Service Planning and Evaluating Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience. Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment. Physical Demands: The work requires mobility and relative physical efforts may be required. Manual dexterity is required in operation of personal computer and other office equipment. Work Environment: Work is performed in a well-lighted, temperature-controlled office setting, utilizing computer terminals, printer and other equipment commonly found in such a setting. ["Clinical Care Coordination: The position provides care coordination to Veterans seeking self-directed, lasting changes aligned with their values. This includes interventions that promote health and wellness and enhance well-being, improve health related outcomes, reduce likelihood of inpatient admission, and improve quality of life. Care Coordinators display unconditional positive regard for their Veterans and a belief in their capacity for change, and honor that each client is an expert on his or her life while ensuring that all interactions are respectful and non-judgmental. This includes: - Serving as a single point of contact for ALS patients and their caregivers to proactively address the complex needs of these patients in the modern paradigm of ALS care. - Coordinating needed services with other specialties within or outside the VA. - Arranging appointments/ensure follow-up at the ALS Interdisciplinary Clinic - Providing ALS educational information, resources and tools to patients and their caregivers. Although facility patient volumes are relatively small, the care management demands of these patients are high (e.g., frequent secure messages, need for expedient actions, supportive care around end-of-life, etc.,) and Care Coordinators interface with the relevant specialty partners (e.g., social work, prosthetics, respiratory care/sleep, GI, interventional radiology, dietician, palliative care, etc.) The Neurology Care Coordinator will also provide a similar level of service for the Movement Disorder and Multiple Sclerosis clinics, as well as other care coordination within the department as assigned. Administrative Support Duties: Serves on Edward Hines VA Hospital department and hospital committees as requested by the service chief. Completes all Medical Record documentation requirements as specified in the Medical Staff Rules and Regulations on a concurrent basis. Attends staff meetings, mandatory training, and complete peer review evaluations. Time will be provided to complete this work. Nature of Assignment: The knowledge required is specialized (to include familiarity with specific types of durable medical equipment, sensitivity in caring for patients with a terminal illness, etc.). Working knowledge of all policies, regulations and guidelines to carry out activities of the Neurological Service. Effective interpersonal, customer service and oral communication skills, including ability to work with diverse populations and build positive relationships. Projects usually take place within organization with related functions and objectives, although organization and work procedures differ from one assignment to the next. Typical organizational efficiency assignments involve observing work in progress to identify and resolve problems in workflow, work methods and procedures, task distribution, overall workload, forms and record keeping, span of control, and organizational structure. When performed, evaluative studies involve measurement of current work output, group productivity and accomplishments, or identification of current resource needs (staff, supplies, equipment, and space). Typically, the employee prepares a narrative report containing a statement of the issue or problem, background, observations, options for change, and recommendations for action. Findings and recommendations are based upon analysis of work observations, review of production records or similar documentation, research of precedent studies, and application of standard administrative guidelines (e.g., staffing guidelines for performance and production. The incumbent plans and carries out projects to improve the efficiency and productivity of organizations and employees in administrative support activities. Incumbent analyzes and makes recommendations to resolve conventional problems and situations in workflow, work distribution, staffing, performance appraisal, organizational structure, and/or administration. Incumbent may be assigned portions of broader studies of largely administrative organizations or participate in the evaluation of program effectiveness at the operating level. Work may also involve developing detailed procedures and guidelines to supplement established administrative regulations or program guidance. Performs other duties as assigned.
