Providence Medical Group is seeking a nurse practitioner to join a care team serving homebound patients throughout Santa Rosa. You will be providing home-based care for Palliative and homebound patients. This means you will be seeing primarily a Geriatric patient population while traveling across the region as a solo practitioner. As such, the role offers a large amount of flexibility in your day-to-day. This is a position that has a huge impact on the overall health and wellbeing of a vulnerable and underserved patient population, and therefore the ideal candidate will be compassionate about the patients they serve.
