The role you’ll contribute:

The RN transplant coordinator is a member of the multidisciplinary team that works together to provide individualized care to patients who are seeking and/or have received a solid organ transplant. This position assumes responsibility and accountability for comprehensive care, support, and education to transplant recipients, living donors, if applicable, and their families throughout all phases of the transplant process. Participates as a member of the transplant multidisciplinary team to identify and meet the physical and emotion needs of the patient, ensuring the coordination of the clinical aspects for the transplant recipient. Acts as a liaison between the kidney transplant program and dialysis facilities, as applicable. Utilizes the nursing process to promote and restore optimal health and well-being of the patient. Ensures continuity of care through effective communication and collaboration with the multidisciplinary team, referring providers, and other health care professionals as necessary. The RN coordinator can cross-cover for other coordinators, multi-task, prioritize, and triage accordingly. Possible travel required to outreach clinics in Colorado and throughout the region. Assumes personal accountability for organizational mission and core values of AdventHealth.

  • Assess patients’ physical, psychosocial, cultural, and spiritual needs through observation, interview, review of records, and clinical data. Evaluates patients’ responses and intervenes in a timely manner to ensure optimal patient outcomes.
  • On call as necessary
  • Evaluations can be done in the hospital, outpatient setting, outreach facilities, or via tele-medicine.
  • Conducts pre-transplant interviews with prospective transplant patients and families. Interpret eligibility criteria for patients, families, physicians, and outside agencies.
  • Assess patients’ compliance and ability to adhere to a medication regimen and treatment recommendations
  • Effectively performs a variety of therapeutic/diagnostic interventions.
  • Serves as a patient advocate.
  • Anticipates needs of provider, staff, patient, and family.
  • Implements patient plan of care and observes outcomes in accordance with nursing standards and physician direction.
  • Prepare and analyze pre-transplant clinical data and collaborate with other medical personnel in preparation of patients and families for transplantation; obtain medical and social histories.
  • Coordinate pre-transplant preparations.
  • Assess patient/family learning style and appropriately educates patients and families regarding all aspects of transplantation and documents understanding.
  • Engages patient/family to foster commitment to comply with plan of care.
  • Collaborates with multidisciplinary team to develop and implement holistic, individualized plan of care.
  • Maintains thorough and confidential maintenance of patient charts and records.
  • Utilizes transplant specific patient database for completion of patient records and documentation.
  • Documents ongoing status, interventions, communications, patient response and outcomes in accurate, timely manner.
  • Evaluates outcomes of patient care and transplant process. Adjusts nursing care as necessary to ensure optim patient care.
  • Coordinates all aspects of transplant process for the transplant patient which may include outreach trips/travel.
  • Facilitates the flow of communication and process regarding UNOS listing, organ placement, recipient admission to the physicians, organ procurement organization, UNOS, hospital staff, and transplant team.
  • Arranges for patient’s ordered diagnostic and therapeutic services as well as consultations.
  • Maintains ongoing inpatient and outpatient coordination of care post-transplant for immediate and long-term follow-up, with 24 hour on-call responsibilities.
  • Effectively uses time, personnel, equipment and supplies to provide high quality, cost effective delivery of patient care
  • Adapts to rapidly changing technologies, policies, regulations, workload and staffing through priority setting, use of sound critical thinking, and flexibility.
  • Professional role model for all staff demonstrating ethical, legal, and professional nursing process.
  • Adheres to professional code of ethics and to all accreditation and regulatory requirements.
  • Maintains high skill level and professional growth through ongoing education, meeting attendance.
  • Communicate with third-party payers regarding issues of medical necessity and benefits coverage for their clients undergoing transplantation if necessary.
  • Pursues and/or maintains certification as Transplant Coordinator.
  • Participates in community-based activities.
  • Compose written transplant summaries, clinic visit summaries, and letters.
  • Conduct post-discharge planning and education for transplant patients.
  • Compile and review post-discharge data for clinic management of transplant patients.
  • Provide direct care to transplant patients in the outpatient transplant clinic.
  • Provide for continuity of care between inpatient admissions and outpatient visits.
  • Collaborate with medical faculty, other transplant coordinators, operating room staff, nursing personnel and other health disciplines to facilitate meeting patient and family needs.
  • Select and present appropriate patients for organ transplantation based on Porter Transplant Center and UNOS guidelines.
  • Provide telephone consultation and follow-up to support patients and their families. This requires the ability to recognize patient problems and symptoms, apply appropriate medical interventions and transplant protocols and interpreting these to community physicians and health professionals involved in the care of the transplant patients.
  • Participate in multidisciplinary meetings and rounds to provide optimal patient care.
Qualifications

The expertise and experiences you’ll need to succeed:

Minimum qualifications:

  • Bachelor’s degree in Nursing
  • Current CO RN License
  • Two years of acute or critical care nursing experience
  • Strong interpersonal, critical thinking, and analytical skills
  • Strong computer skills

Preferred qualifications: 

  • Previous Transplant experience
  • Clinical Transplant Coordinator certification
