Overview Atlantic Visiting Nurse, part of Atlantic Health System, is seeking a per diem Registered Nurse for the Home Care and Hospice Care Access Department to work weekdays and occasional weekends as required.Responsibilities Essential Functions/Responsibilities:Perform as a contributing member of the Care Access Center for Atlantic Visiting Nurse, by accepting referrals for a Home Care and Hospice.Partner with Case managers, social workers, physicians, family members and patients, accepting referrals and obtaining the most accurate clinical information, and physician documentation to complete a referral for a Home Care or Hospice admission.Must have regulatory knowledge of Home Care, and obtain documentation of the same.Must u
