The Geri PACT Physician is assigned to the Geri PACT clinic Section of Geriatrics Rehabilitation and Extended Care (GREC), VA Pacific Islands Healthcare System (VAPIHCS). VA provides patient centered care, delivered by engaged, collaborative teams in an integrated environment that supports learning and continuous improvement. The Geri PACT Physician must have outstanding clinical and interpersonal skills as a member of the VA professional health care team and provide continuous veteran-centered care and services to Veterans and their family members.The Geri PACT Physician provides outpatient primary and geriatric consultation services to Veterans and provides clinical support to the Geri PACT Team Members under the facility outpatient Geri PACT Program.
