Provide competent psychosocial care to patiets admitted to the hospital and to outpatients in the community. Organize and facilitate support group(s) as needed that respond to the needs of the cancer community. Serve as Social Worker on Palliative Care Team and participate in the working/steering committee of the Pallative Care Program. Responsibilities: Case Finding: Identify eligible cancer patient or their families through multidisciplinary team meeting, Physician referrals, or self-referral. Develop case finding methodologies and pathways. Publicity and patient education materials: Develop support program promotional materials.
