The role you’ll contribute (Job Summary):
Plans, implements and organizes social work resources and evaluates and provides treatment regarding mental health issues for patients; linkage to resources in the community.

Assess patient’s physical, psychosocial, cultural and spiritual needs through observation, interview, review of medical records & clinical data.
Develop and implement patient plan of care and observe outcomes in accordance with social work standards and in collaboration with the interdisciplinary team.
Manages resources and patient flow through matching clinical competencies with individualized patient’s and unit needs.
Use quality measures to improve performance and accountability for patient outcomes, patient experiences and safe delivery of care.
Qualifications

KNOWLEDGE AND SKILLS REQUIRED:

EDUCATION AND EXPERIENCE REQUIRED:
·Education: Master of Social Work (MSW)
EDUCATION AND EXPERIENCE PREFERRED:
·6 months home care/hospice experience
·Two years post Master’s experience in health care or related field
LICENSURE, CERTIFICATION OR REGISTRATION REQUIRED:
·Cardiopulmonary Resuscitation (CPR)
·Current Basic Life Support (BLS) from American Heart Association for facilities with an Automated External Defibrillator (AED), preferred for facilities without an AED
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED:
·Licensed Clinical Social Worker (LCSW)
