WHO WE ARE LOOKING FOR:

Franciscan Physician Network is seeking a Family Medicine/Geriatric Nurse Practitioner to work PRN & join our PACE Program team at Franciscan Senior Health & Wellness in Indianapolis, IN to help meet the growing demands of our outpatient program.

WHAT WE NEED:

Ideal candidate will have a seasoned nursing home care, family medicine and/or geriatric background. Candidate would thrive in a fast paced, autonomous, self-managed environment. Provider will provide direct medical care as needed to participants in the home, the PACE center, and in other health care settings. Provider will collaborate with our PACE Medical Director and PACE Program team to coordinate and deliver the highest quality of patient care. Candidate will also have desire to develop strong ties with patients and the community in which they serve.

WHAT YOU CAN EXPECT:

• Practice Hours: Monday - Friday, 8:00 a.m. - 5:00 p.m.
• Traditional PRN schedule
• Call responsibilities based on provider staffing; minimum 1:3 rotation
• Average outpatient volume: 4 - 6 patients per day
• On-site training required in Indianapolis for up to 6 weeks
• Daily Duties:
  • Care planning with the PACE program's interdisciplinary team
  • Day-to-day care of patients
  • Manage and round on PACE participants admitted to long term care facilities or acute care in participant facilities
  • Review medical histories and medical assessments
  • Implement plan of care with patient and family
• Outreach potential with our Lafayette location
• Will work with skilled and long-term patients

Qualifications:

• Master's Degree - Required
• Nurse Practitioner (NP) or Nationally Recognized Certification - Required
• Basic Life Support Program (BLS) - American Heart Association - Required
