Lee Health Heart Institute is looking for an Advanced Practice Provider (APRN or PA) with a passion for Cardiology.
The Heart Care Transition Clinic is a specialized medical office for cardiac patients recently discharged from the hospital to receive follow-up medical care. The Heart Care Transition team works with your cardiologist to ensure patients get the follow-up care needed in a timely manner. These clinics focus on patients with a diagnosis of Heart Failure, Afib and other chronic cardiac conditions to provide follow-up care, medication and diagnostics review and focuses on in-depth patient education as a means to assist patients with diet and lifestyle changes as a means to liaise care back to the patient primary cardiologist and mitigate readmissions.
This position is primarily based out of our HealthPark Medical Center in Fort Myers, however training and orientation will require floating to all 4 of or acute care facilities.
Schedule: Monday - Friday, 8 hour shifts (May consider 4 day work week of 10 hour shifts for the right candidate). No Call, No Weekends!
Basic Position Requirements:
Graduate of a accredited Advanced Practice Registered Nurse or Physician Assistant program.
Board Certified based on Advanced Training
Basic Life Support
Active, Unrestricted License in the State of Florida (or ability to obtain prior to start)
Preferred: Minimum 1 year of Advanced Provider Experience in the field of cardiology
