SNF (Skilled Nursing Facility Rounder) Physician
The Southeast Permanente Medical Group, Inc. (TSPMG) has an outstanding job opportunity available for a provider who will care for Kaiser Permanente (KP) patients in the Skilled Nursing Facility (SNF) setting. The provider will manage the care of the member, collaborate with other KP colleagues on the SNF rounding team, and interact with the SNF facility staff. The provider will manage members across facilities with the assistance of an NP and Acute Transitional Case Manager. They will escalate facility issues and patient care problems to the facility staff and appropriate KP staff in a timely manner.
