SummaryThe clinical and program activities assumed under this position are conducted as part of the Home-Based Primary Care (HBPC) program. The HBPC Social Worker is a core member of the HBPC interdisciplinary team. The primary responsibilities of this position are to provide comprehensive clinical assessments.
