Job Description Responds to calls from triage and provides patient visits on request of designated triage nurse Responds to medical or emotional crisis and/or death as requested by designated triage nurse Provides skilled nursing care focusing on pain control and symptom management Conducts hospice admissions as needed Consults appropriately with attending physician and medical directors to coordinate patient care and receive orders Assists RNCMs with their patient assignments by conducting scheduled after-hours visits as necessary to meet patient care needs.
