Job Description Responsibilities: Develops, coordinates, implements and evaluates the plan of care with patients s/p CABG, Valvular repair or replacement, MI, PTCA with or without STENT placement AND Stable angina. Instruct patients on appropriate low-level activity as per department policy & procedure. Evaluates patients cardiac status before, during and after exercise, including instruction and supervision of a structured progressive monitored activity program for all candidates. Organizes and conducts patient and family groups for the purpose of supportive counseling, exercise limitations and coronary artery disease
