Nurse Practitioner- Pediatric
Full Time, 32 hrs. per week, Day Shift
4 days/week
Day off each week will be Tuesday, Wednesday or Thursday. MUST work Mondays and Fridays.
No weekends or holidays
Potential for very minimum on- call in the future
This role will see patients anywhere in our service territory
Orientation - Must be able to do a full time (40hr/week) orientation for 1-2 months
Functions:
Serves as a member of the Sunflower Pediatric Hospice and Palliative Medicine Interdisciplinary Team (IDT).
Provides critical thinking and clinical judgment to address the physical, emotional, psychological, and spiritual needs and reflect the goals of care of the patients and families receiving hospice and palliative medicine services.
Provides clinical direction and expertise to the Sunflower IDT.
Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted palliative care and Choices Healthcare standards and guidelines.
Collaborates with Choices Healthcare physicians and/or Pediatric Medical Director and the Sunflower IDT in admission, certification, recertification, and discharge decisions.
Collaborates and maintains communication with the patients’ healthcare team (e.g. primary care provider, subspecialty teams, and hospice and palliative medicine physicians) as needed for the coordination of hospice care and palliative medicine services.
Monitors plans of care to assure consistency with Choices Healthcare philosophy, policies, procedures, and protocols.
Provides clinical evaluations for hospice and palliative medicine patients when necessary and requested as part of the plan of care, and documents visits and services according to Medicare and Choices Healthcare standards.
Provides support and education to patients, caregivers and facility staff and acts as a liaison to facilitate goals of care.
Participates in interdisciplinary team meetings as scheduled.
Participates in learner education (e.g. medical student, residents, and fellows) as needed.
May be required to participate in some adult hospice responsibilities as directed by supervisor PRN.
NP Pediatric Requirements:
Certified Nurse Practitioner license in the Commonwealth of Pennsylvania as Pediatric Nurse Practitioner (PNP) or Family Nurse Practitioner (FNP).
Certification in Hospice & Palliative Medicine required within three years from date of hire.
1 year of pediatric experienced required
Prescriptive authority and unrestricted DEA license required
Home Hospice - this role involves seeing patients anywhere in our service territory
PT8 or 64 hrs. per two week pay period
8am - 5pm
Potential for "on call" in the future
Essential Functions
Assists with the provision of palliative medicine in accordance with organizational, state, and federal regulations. Serves as a comprehensive direct care provider with demonstrated skilled proficiency in symptom management and advanced illness care.
Provides critical thinking and clinical judgment to address the physical, emotional, psychological, and spiritual needs and reflect the goals of care of the patients and families receiving palliative medicine services.
Provides clinical direction and expertise to the interdisciplinary team. Participates in goals of care discussions and development of plans of care that are clinically appropriate and that meet accepted Choices’ standards and guidelines.
Provides comprehensive clinical and psychosocial evaluations of patients and their families in relation to their goals of care and serves as a patient advocate in the development of patient centered goals of care.
Collaborates and maintains communication with the patients’ attending providers and sub-specialists for the coordination of palliative medicine services.
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities and monitors the effectiveness of interventions.
Provides cross coverage to hospice services on an as needed basis including in person visits.
NP Palliative role requirements:
